• FOR DOCUMENT VERIFICATION AND ADMISSION, THE CANDIDATES HAVE TO BRING THE FOLLOWING –

1. ORIGINAL DOCUMENTS –

a) Admit card of NEET

b) Admit card of HSLC or Equivalent Examination

c) Pass certificate of HSLC or Equivalent Examination

d) Marksheet of HSSLC or Equivalent Examination

e) Pass certificate of HSSLC or Equivalent Examination

f) Permanent Residential Certificate (PRC)

g) Caste Certificate (wherever applicable)

h) Non-Creamy Layer Certificate for OBC/MOBC including TGL/Ex-TGL/Motak/Moran/State Char Area Community (Annexure-III)

i) Category Certificate (wherever applicable)

j) All Annexures in Original as per format available in the website of DME, Assam (for State quota candidates only)

2. Two (02) sets of photocopies of the above original documents – self attested

3. Six (06) Passport size photos of candidate.

4. Admission fees – Rs. 24,000/- and Counselling fees – Rs. 1000/- (Total 25,000/-)
